Weekly Reports of Fatalities, Catastrophes, and Other Events

The Occupational Safety and Health Administration (OSHA) Standard, 29 CFR Subpart 1904.39, Reporting Fatality, Injury and Illness Information to the Government requires that within eight (8) hours after a work-related death of any employee or the in-patient hospitalization of three or more employees, employers must orally report the fatality/multiple hospitalization by telephone or in person to the OSHA Area Office or to the State Plan Office that is nearest to the site of the incident. Employers may also use the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), to report this information.

OSHA has created a Web site that provides information on worker injuries as required above and reported by businesses, titled:

            Weekly Reports of Fatalities, Catastrophes, and Other Events.

There are three sections to the site that should be of interest to risk managers and loss control consultants.

The first section, the opening page, Recent Weekly Reports of Fatalities and Catastrophes, only displays a table that provides links to weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers as listed in sections two and three. The summaries include only preliminary information, as reported to OSHA Area Offices or to states which operate OSHA-approved state plans. OSHA investigates these incidents and once the OSHA investigation is complete, these summary reports will be updated with webpage links to their corresponding inspections and citation information.

The second section, Weekly Fatality/Catastrophe Report, is a table that contains a specific week's summaries. It lists the data by OSHA region, identifies the business name, date of the incident, and a preliminary description of the incident.

The third section, Regional Federal and State Fatality/Catastrophe, is a table that provides totals, for a specific week, of fatalities and catastrophes, tabulated by region and showing an FY year –to-date total.
